Diabetes Screening in Malaysia: Who Should Test, Which Test and What Your Numbers Mean

Your mum's last check-up was "fine", but nobody remembers which test she did or what the number was. Sound familiar?

Diabetes screening is a simple blood test that checks your sugar level before any symptoms show up. It takes a few minutes, and in Malaysia it is often free. With World Diabetes Day 2026 around the corner, here is who needs diabetes screening, which test to ask for, and what the numbers mean.

Who should get diabetes screening?

Anyone with symptoms like constant thirst, peeing a lot or feeling tired after meals should get tested now. But many people with high blood sugar feel nothing at all. You should ask for diabetes screening even without symptoms if you:

  • Are over 25 and of Chinese or South Asian background, which the NHS lists as a higher-risk group
  • Have a parent, brother or sister with diabetes
  • Carry extra weight around the tummy, or sit most of the day
  • Have high blood pressure or high cholesterol
  • Had diabetes during a pregnancy

3 common diabetes screening tests

  • HbA1c — shows your average blood sugar over the last two to three months. No fasting needed, so it is the easiest to fit into a busy day.
  • Fasting blood glucose — taken in the morning after at least 8 hours with nothing but plain water. Skip the kopi O too.
  • Oral glucose tolerance test (OGTT) — blood is taken before and two hours after a sweet glucose drink. It picks up people whose sugar only rises after eating.

A finger-prick test at a pharmacy or health fair is a useful first check. It cannot confirm diabetes on its own, so follow up a high reading with a proper lab test.

How to read your diabetes screening results

These are the cut-offs used by the American Diabetes Association, converted to the mmol/L units used in Malaysia:

  • HbA1c: normal below 5.7% · prediabetes 5.7–6.4% · diabetes 6.5% or higher
  • Fasting glucose: normal below 5.6 · prediabetes 5.6–6.9 · diabetes 7.0 mmol/L or higher
  • 2-hour OGTT: normal below 7.8 · prediabetes 7.8–11.0 · diabetes 11.1 mmol/L or higher

One high number is usually repeated on another day before a doctor confirms diabetes. Your doctor may also use slightly different prediabetes ranges, so ask them to explain your own result.

Where to get diabetes screening in Malaysia

You don't need to pay much, or anything at all:

  • Klinik Kesihatan — the Health Ministry's National Health Screening Initiative has checked over 2.65 million Malaysians under 40 since 2022
  • PeKa B40 — free screening at private panel clinics for lower-income Malaysians aged 40 and above
  • Private clinics and hospitals — most offer an HbA1c or fasting glucose test as part of a basic health screening package
  • Workplace and community health days — often include a finger-prick check

Those screening figures come from the Health Minister's update in Parliament, which found about 176,000 cases of high blood glucose among those under 40. Many of them would never have known otherwise.

What to do after diabetes screening

  • Normal: keep your habits and repeat diabetes screening as your doctor advises, sooner if you have several risk factors.
  • Prediabetes: this is your early warning. The ADA suggests checking again every one to two years, and small changes now, like fewer sweet drinks and a walk after dinner, really count.
  • Diabetes: don't panic. Follow your doctor's plan, take your medication as prescribed and keep your follow-up appointments.

Got a borderline diabetes screening result?

Many people with a borderline result tell us the hardest habit to change is their sweet morning drink. That's when some swap it for one that supports healthier blood sugar.

Regen Cocoa by Hishin is a chocolate drink made for people watching their blood sugar. It combines two plant ingredients:

It is KKM, MeSTI and GMP certified, 100% plant-based and suitable for vegetarians. Each serving has only about 0.3 g of natural sugar and around 95 kcal, yet it tastes like a smooth chocolate drink, not bitter and not "medicine-like".

Mix 2 tablespoons with 150–200 ml of room-temperature or warm water and drink it before breakfast. It makes an easy swap for your usual kopi, teh tarik or 3-in-1.

Regen Cocoa works together with the habits above, not instead of them, and not instead of the treatment your doctor prescribes.

The bottom line

Diabetes screening is quick, cheap and often free in Malaysia, and it can catch high blood sugar years before symptoms appear. Ask for an HbA1c or fasting glucose test, write down your number, and bring a family member along for theirs.
